Three steps to intake

A simple path from referral to services

You do not need to determine final PRP eligibility before making a referral. Send the available information, and BMORE PRP will guide the client and referring provider through the remaining requirements.

Submit the Referral

Complete the secure referral form with the client’s basic demographic, insurance, clinical, and provider information.

Review and Schedule

Our intake team reviews the referral, verifies coverage, contacts the client, and coordinates any missing information with the provider.

Complete Intake and Begin

The client meets with a Rehabilitation Coordinator to complete assessments, identify goals, and prepare for service authorization and assignment.

  • Community Rehabilitation Worker assignment

Provider note: Clinical records may be submitted as supplemental information. BMORE PRP will contact the referring provider if additional documentation, signatures, or clarification are needed.
